What to Expect at Orthodontic Appointments UK

Knowing what happens at each stage of orthodontic treatment removes the anxiety of the unknown. This guide explains every appointment type — from your first consultation to the day your braces come off — including what is done, how long it takes, and what to prepare, so you arrive informed and confident at every visit.

Your first consultation — what actually happens

The first appointment is a clinical assessment, not the start of treatment. The orthodontist will examine your teeth, gums, and bite, take or review X-rays (usually a panoramic OPG and lateral cephalogram), and carry out a visual inspection of your face, jaw, and smile. You will not have braces fitted at this appointment. The orthodontist will discuss what they have found, outline possible treatment options, explain the expected duration and costs, and give you time to ask questions. This appointment typically lasts 30–60 minutes. You may be asked to fill in a medical history form beforehand, which covers medications, allergies, and relevant medical conditions. Bring a list of any medications you take and be prepared to mention any jaw pain, headaches, or dental sensitivity.

The records appointment — scans, photographs, and X-rays

Before treatment can begin, the orthodontist needs a full set of clinical records to plan treatment precisely. This appointment includes digital intraoral scans (or traditional impressions) of your upper and lower teeth, a full set of clinical photographs (face and intraoral), and X-rays if not already taken. The records appointment typically takes 45–60 minutes. Nothing is fitted at this stage — the scans and photos are used to produce study models and, for aligner treatment, the digital treatment simulation (ClinCheck or equivalent). You may have a gap of 2–6 weeks between the records appointment and the fitting appointment while the orthodontist plans treatment and any custom appliances are manufactured.

Brace fitting or aligner handover — what to expect on day one

The fitting appointment is when treatment begins. For fixed braces, each bracket is bonded individually to the front surface of your teeth using a dental adhesive cured with a light. Archwires are then threaded through the brackets and secured with ligature ties or self-ligating clips. The appointment takes 60–90 minutes. Your teeth will not feel painful during fitting, but aching and soreness typically starts 4–6 hours afterwards and peaks around 24–48 hours as your teeth begin to respond to the initial forces. Soft foods for the first few days are advised. For aligner treatment, the aligner handover appointment involves fitting any necessary attachments (small tooth-coloured composite bumps that help aligners grip and move teeth), interproximal reduction (IPR — gentle filing between teeth to create space) if planned, and instruction on how to insert and remove aligners correctly. You will receive your first batch of aligner trays to take home.

Adjustment appointments — how often and what happens

Fixed brace adjustment appointments are typically scheduled every 6–10 weeks. At each visit, the orthodontist or orthodontic therapist checks progress, removes the current archwire, and replaces it with the next wire in the treatment sequence — usually progressing from thinner, flexible wires in early treatment to thicker, stiffer wires that apply more precise tooth control later. Elastic modules (the small coloured or clear rings securing the wire) are replaced. Some appointments include additional components such as inter-arch elastics (rubber bands worn between upper and lower teeth), separating elastics, or the attachment of additional auxiliaries. Appointments last 20–40 minutes. For aligner patients, check-up appointments are less frequent — every 6–12 weeks — and involve reviewing tooth movement against the digital plan, fitting new attachment trays if needed, and issuing the next batch of aligner sets.

Retainer fitting — the appointment after braces come off

The debond appointment removes your brackets and wires (for fixed braces) using debonding pliers and polishing equipment. Your teeth will feel smooth, slightly sensitive, and unfamiliar. Retainers are fitted the same day or within days. Standard retention involves a fixed bonded wire behind the lower front teeth (and sometimes upper teeth), plus removable clear retainers worn overnight. The retainer fitting appointment takes 30–60 minutes depending on what type of retention is provided. You will be shown how to clean around the fixed wire and how to store and wear your removable retainers. Retainer wear instructions vary by clinician — most recommend nightly wear indefinitely, not just for a few months. Skipping retainer wear causes teeth to drift back.

Emergency appointments — what counts and what to do

Orthodontic emergencies are less dramatic than they sound. A poking wire, a loose bracket, a lost aligner tray, or a broken archwire are common issues that cause discomfort but are rarely urgent in a medical sense. Most practices offer emergency slots within 24–48 hours for genuine discomfort. A soft wax bar is provided at the fitting appointment — pressing orthodontic wax over a poking wire or broken bracket provides immediate relief until your appointment. Contact the practice promptly if a bracket or band is completely detached, a wire has shifted significantly, or you have significant swelling or pain that is not typical soreness. Lost or cracked aligner trays should be reported immediately, as leaving a gap in the aligner sequence can cause teeth to drift off the treatment plan.

Online orthodontic assessment — what you can do before your first appointment

A guided photo assessment via Virtual Consultant Online lets you submit photos of your teeth from home for clinician review, receiving an indication of likely treatment options and costs before committing to paid in-person consultations. This is particularly useful for patients comparing multiple practices, those who are anxious about attending in person before knowing what to expect, and those who want to understand their options in advance. The photo assessment does not replace the clinical records appointment — X-rays and intraoral scans cannot be done remotely — but it gives you a head start on understanding what orthodontic treatment is likely to involve for your specific situation.
