Orthodontic Support Hub

True orthodontic emergencies — such as severe pain, significant swelling, or a wire causing deep tissue injury — should be reported to your clinic immediately. Most brace problems like broken brackets, loose brackets, and protruding wires are urgent but not medical emergencies, and can be managed temporarily at home while you await clinical advice.

If a bracket has come off or a wire has snapped, leave the bracket on the wire if it is not causing harm, apply orthodontic wax over any sharp edges, and report the issue online through Virtual Consultant Online. A clinician will review your photos and advise whether you need an urgent repair.

Yes — the support request tool on Virtual Consultant Online lets you upload photos of the affected area and describe what has happened, so a registered clinician can see the problem and advise on next steps, usually within 24 hours on working days. Photos matter because they let a clinician judge urgency without you attending in person: whether a bracket needs re-bonding soon or can wait, or whether a wire end is genuinely injuring tissue. Take them in good daylight, gently retract your lip or cheek, and focus on the specific bracket, wire, or tooth causing trouble.

No — most brace problems are urgent rather than emergencies. Broken or loose brackets, a wire that has slipped out of a bracket, and mild soreness after an adjustment can usually be managed at home for a short period with orthodontic wax and soft foods while you wait for advice. They should still be reported promptly so a clinician can judge urgency and arrange a repair. Genuine emergencies are different: severe pain, facial swelling, bleeding that will not stop, difficulty breathing or swallowing, or a wire embedded in the gum or cheek all need same-day contact with your practice, NHS 111, or emergency dental services.

Apply a small piece of orthodontic wax over the protruding wire end to prevent it catching your cheek or gum. You can also try gently tucking the wire back with the eraser end of a clean pencil. If it cannot be managed, report it online and a clinician will advise.

Call your clinic directly if you are in severe pain, have swallowed part of your brace, have facial swelling, or if a wire is causing significant injury that cannot be controlled with wax. NHS guidance is to contact your dentist or NHS 111 for urgent dental problems, and to go to A&E or call 999 for severe swelling that affects breathing or swallowing, or bleeding you cannot stop. For non-emergency issues, the online support tool is the fastest route.

Pick the support topic on this hub that comes closest to what you are experiencing — broken brace, loose bracket, wire sticking out, or orthodontic pain — and read that page first, as it explains what to do immediately and how urgent the problem is likely to be. If none of them quite matches, submit a support request describing the symptoms in your own words and upload photos of the affected area, following the photo guide. You do not need to identify the problem correctly yourself; a clear photo lets a registered clinician work out what is happening without an in-person visit.

Yes — a broken or loose bracket stops applying force to the tooth it was bonded to, so movement on that tooth pauses until it is repaired. A wire that has slipped out of a bracket or become distorted can do the same, and in some cases lets a tooth drift back towards its previous position. How much this matters depends on which tooth is affected and what stage of treatment you are at, but the pattern is consistent: the longer a breakage goes unreported, the more likely it is to add weeks to the overall timeline. Report problems within a day or two and follow your clinician's advice on urgency.
