What clear aligner refinements are
A refinement is an additional set of aligners prescribed after the initial aligner series has been completed or when teeth are not tracking as planned mid-treatment. After completing all aligners in an initial series, your clinician takes a new scan of your actual tooth positions and compares them to where the ClinCheck (treatment plan) predicted they should be. Any gap between actual and planned position is modelled into a new series of refinement aligners to finish the job. Refinements are a standard part of comprehensive aligner treatment — they are not a failure of treatment.
Why teeth may not track as planned
Clear aligner 'tracking' refers to how well teeth follow the planned movements in each aligner. Non-tracking is common and has several causes: insufficient aligner wear time (under 20–22 hours/day), attachments that have come off or are ineffective, teeth that move slower than predicted (biological variation), complex rotation or root movements that aligners handle less predictably, and patient non-compliance with elastics. If aligners are consistently not tracking after multiple trays, your clinician may pause, reassess, and prescribe an early refinement.
How many refinements are typical
For Invisalign Comprehensive: multiple refinements are included in the original fee with no limit (under the 'unlimited refinements' protocol). For most other systems (Spark, SureSmile, ClearCorrect) comprehensive packages typically include at least one or two refinement rounds. Short-course packages (Invisalign Lite, most DTC systems) typically include no refinements or only one. A typical comprehensive Invisalign case may use 1–3 refinement rounds. Unusually complex or non-compliant cases may use more.
What happens during a refinement
At a refinement appointment, your clinician takes new intraoral scans of your current tooth positions. These are submitted to the aligner lab (Invisalign, Spark, etc.), which produces a new ClinCheck showing the movements still needed. You and your clinician review and approve the new plan. New aligners are fabricated and shipped — this typically takes 2–4 weeks. You continue wearing your existing retainer or final aligner from the previous series until the new aligners arrive.
Refinements and the impact on total treatment time
Each refinement round adds time: 2–4 weeks of fabrication plus however many weeks of wearing the new series. A single refinement round typically adds 2–5 months to treatment. Multiple refinements can extend treatment by 6–12+ months beyond the original estimate. This is why strong compliance from the start — consistent 22-hour wear, keeping all appointments, maintaining attachments — significantly reduces the likelihood and number of refinements needed.
What to check in your treatment contract about refinements
Before signing for clear aligner treatment, ask specifically: How many refinements are included in the fee? Is there a time limit on included refinements (some practices include refinements only within 24 months of starting)? What is charged if additional refinements are needed beyond what is included? What triggers the clinician to prescribe a refinement vs progressing to retainers? When will retainers be made — after the final refinement only? Getting clear written answers to these questions prevents surprise costs.
