Bruxism and Braces UK

Bruxism can affect appliance wear, breakages, jaw comfort, tooth wear, and retention planning during and after orthodontic treatment.

How does teeth grinding affect fixed braces?

Heavy nocturnal bruxism — grinding or clenching the teeth during sleep — places lateral and vertical forces on orthodontic brackets and wires that are not part of the planned tooth-moving mechanics. Repeated grinding forces can contribute to bracket debonding, particularly on the lower anterior teeth where the grinding contact is heaviest. Bite turbos, which are small composite ramps often placed on the lower incisors to open the bite during deep-bite treatment, can be worn smooth or sheared off by sustained grinding. Attachments used with clear aligner treatment can similarly be dislodged. Bruxism does not necessarily prevent orthodontic treatment, but the orthodontist should be informed so that the bonding approach, appliance design, and appointment frequency can be adjusted to account for the higher breakage risk.

Can clear aligners be used as a bruxism guard?

Aligners provide a thin plastic barrier between the upper and lower teeth while worn, which may reduce direct tooth surface wear from grinding. However, they are not designed as bruxism occlusal splints and should not be used as a substitute for a properly designed night guard. Heavy grinders often wear through aligner trays faster than the planned treatment schedule allows — typically one to two weeks per tray — necessitating more frequent changes or accelerated progression. If you grind heavily during sleep, your orthodontist needs to know in advance because the aligner wear schedule and total tray count may need to be adjusted. A dedicated hard-acrylic bruxism splint designed to dissipate grinding forces is a different device from a thin aligner tray.

Can you wear a night guard while having fixed braces?

Standard prefabricated or custom-made hard acrylic night guards do not fit over fixed brackets and cannot be used with fixed brace treatment. The brackets protrude from the tooth surface and prevent a guard from seating properly. This leaves bruxism patients with limited protection for their natural teeth and appliances during fixed brace treatment. Some orthodontists can fabricate a soft vacuum-formed cover over the brackets that provides minimal cushioning, but this is not a true bruxism guard. Patients with severe confirmed bruxism should discuss the interaction between grinding and their planned treatment before starting, as this may influence the decision between fixed braces and clear aligners.

How does bruxism affect retention planning after orthodontic treatment?

Retention after orthodontic treatment is already a lifelong requirement, but bruxism adds specific challenges. Removable retainers — whether Hawley or vacuum-formed — can be worn down or cracked by heavy grinding. Fixed bonded retainers on the tongue surface of the front teeth are at risk of fatigue fracture from the repeated biting forces of a bruxist. Post-treatment retention planning for bruxism patients often involves combining a fixed retainer with a custom-made hard acrylic night guard that fits precisely over the post-treatment dentition. This guard performs two functions: it maintains the teeth in their corrected positions (acting as a retainer) and dissipates the grinding forces (acting as a bruxism splint). The design and review of this combined appliance should involve both the orthodontist and, if severe, a restorative dentist or specialist in occlusion.

Does bruxism cause jaw pain that orthodontics can help?

Bruxism-related jaw pain is mediated through multiple mechanisms — muscle hyperactivity and fatigue, joint loading, tooth sensitivity from wear, and soft tissue trauma. Whether orthodontic treatment helps depends on the cause. If the grinding is partly triggered by an occlusal interference — a tooth contact that forces the jaw into a strained position — correcting the bite may reduce the trigger. However, bruxism is primarily a sleep-related neuromuscular behaviour and orthodontics is not a treatment for the condition itself. Patients who expect orthodontic treatment to stop grinding-related jaw pain should be counselled carefully before starting, as the evidence for orthodontics resolving bruxism or its symptoms is limited. Management of sleep bruxism typically involves a night guard, stress management, and in some cases referral to a sleep medicine specialist.

Should bruxism be managed before starting orthodontic treatment?

Active, uncontrolled bruxism with significant tooth wear should ideally be assessed and a management plan discussed with your dentist or orthodontist before commencing orthodontic treatment. Severe ongoing bruxism without a protective splint can cause tooth surface loss that alters the treatment plan. If the bruxism is well controlled with a night guard, treatment can usually proceed. For mild or moderate bruxism, treatment can begin with the orthodontist factoring in the higher risk of breakages and the need for robust retention planning. Inform your orthodontist of any jaw pain, tooth sensitivity on waking, headaches, or previous advice from a dentist about grinding, as these are all relevant to the planning and management of your case.
