## What white spots are

White spot lesions are areas where enamel has lost minerals, usually because plaque sat around brackets. They often look chalky, matte or opaque, especially when teeth are dry.

## Why fixed braces raise risk

Brackets, wires and elastics create plaque traps. Frequent fizzy drinks, sports drinks, snacking, poor brushing and low fluoride exposure make demineralisation more likely.

## Prevention during treatment

Brush around every bracket twice daily with fluoride toothpaste, clean under wires with interdental brushes, reduce sugar frequency and attend hygiene visits if advised.

## Treatment after braces

Options include remineralisation, fluoride varnish, resin infiltration, microabrasion, whitening or composite bonding. The right option depends on depth, activity, colour and whether enamel has cavitated.

## Whitening timing

Whitening may reduce contrast in some cases but can also make spots stand out initially. It is usually planned after gums settle and enamel rehydrates after debond.

## When it may be decay

Rough, brown, sensitive or cavitated marks need dental assessment. These are not just cosmetic white spots and may need decay management.

### Frequently Asked Questions

Are white spots permanent?

White spots after braces are not always permanent. Early, shallow demineralisation can improve over several months as saliva and fluoride redeposit minerals into the enamel surface, and the appearance often softens once the teeth rehydrate in the weeks after the brace is removed. Deeper lesions that have lost more mineral tend to stay visible and may need resin infiltration, microabrasion or composite bonding to disguise them. Have them assessed by a dentist before deciding anything, because the right approach depends on how deep the lesion is and whether the enamel surface is still intact.

Did braces cause them?

White spots are caused by plaque sitting undisturbed against enamel, not by the brackets themselves. Fixed braces make them far more likely because brackets, wires and elastic modules create traps that are difficult to brush, so acid produced by plaque bacteria draws minerals out of the enamel beside them. Frequent sugary or acidic drinks, snacking between meals and low fluoride exposure all raise the risk further. That is why the marks so often appear as a halo around where each bracket sat, visible from the day the brace comes off.

Can whitening remove them?

Whitening does not remove white spot lesions. It lightens the surrounding enamel, which can reduce the contrast so the marks look less obvious, but in the early stages it often makes them stand out more before the overall shade evens out. Whitening is normally deferred until the gums have settled and the enamel has rehydrated after the brace is removed, and any active decay should be treated first. Discuss it with your dentist rather than buying a shop kit, since in the UK tooth whitening must be carried out or prescribed by a dental professional.

Does resin infiltration help?

Resin infiltration can noticeably improve the appearance of selected white spot lesions where the enamel surface has not broken down. A low-viscosity resin is drawn into the porous enamel after the surface layer is conditioned, changing how light passes through the lesion so it blends with the surrounding tooth. It works best on shallow, non-cavitated marks and is minimally invasive, because little or no healthy tooth structure is removed. It does not suit every lesion, and deeper or brown-stained areas may still need microabrasion or composite bonding. A dentist should assess suitability first.

Can aligners cause white spots?

Clear aligners can contribute to white spots, though the risk is generally lower than with fixed braces because there are no brackets trapping plaque and the trays come out for cleaning. The main danger is drinking anything other than water while aligners are in, since sugary or acidic liquid is then held against the enamel with little saliva to wash it away. Putting trays back in without brushing has a similar effect. Remove aligners for everything except water, brush before reinserting them, and clean the trays daily.

How do I prevent them?

Preventing white spots comes down to removing plaque from around every bracket twice a day using a fluoride toothpaste of at least 1,450 ppm, and cleaning under the wire with interdental brushes. How often sugar is consumed matters more than the total amount, so cut back on snacking, fizzy drinks and sports drinks between meals. Attend hygienist appointments if your orthodontist advises them, and ask whether a higher-strength fluoride toothpaste, mouthwash or varnish would help. Report any chalky marks you notice at your next adjustment rather than waiting until debond.
