# Speech and Braces UK

Braces and aligners can temporarily affect speech, especially when appliances change tongue space or sit behind the teeth.

## Why speech changes

Appliances alter tongue contact, airflow, and space around the front teeth. S sounds are often most affected at first.

## Lingual braces

Lingual braces sit behind the teeth, so speech changes are more common initially. Many patients adapt, but public speakers and performers should discuss timing carefully.

## Clear aligners

Aligners can cause a mild lisp for a few days with new trays. Speaking practice, reading aloud, and consistent wear usually improve adaptation.

## When to seek advice

Sharp rubbing, poor aligner fit, appliance breakage, or speech problems that do not improve should be reviewed.

### Frequently Asked Questions

How long does a braces lisp last?

Most speech changes caused by braces settle within a few days to two or three weeks as the tongue adapts to the new surfaces in the mouth. Fixed braces on the outside of the teeth cause the least disruption. Lingual braces sit behind the teeth, exactly where the tongue makes contact for s, t, d and l sounds, so they cause the most noticeable lisp and often take several weeks to settle. Clear aligners typically cause a mild lisp for a few days after each new tray. Reading aloud speeds up adaptation.

Can I give presentations with aligners?

Presenting, teaching and taking calls with clear aligners in place is realistic for most people, because any lisp is usually mild and fades within a few days of starting a new tray. If an important presentation is coming up, practise aloud beforehand, keep water to hand since aligners can make the mouth feel drier, and where possible change to a new tray several days ahead rather than the night before. Removing aligners to speak is possible but eats into the 20 to 22 hours of daily wear that treatment depends on.

Do braces permanently change speech?

Permanent speech change from orthodontic treatment is uncommon. The tongue adapts to appliances within days or weeks, and once braces are removed speech normally returns to how it was, sometimes improving where a large overjet, open bite or crossbite had been affecting particular sounds. If a lisp or other difficulty persists beyond a few weeks, or is getting worse rather than better, raise it with your orthodontist so appliance fit, a rubbing bracket, a poorly seated aligner or a tongue habit can be checked. Referral to a speech and language therapist is occasionally appropriate.

Which type of brace affects speech the most?

Lingual braces, by a clear margin, because the brackets sit on the tongue side of the teeth exactly where the tongue makes contact for sounds such as s, t, d and l. A lisp is common for the first few weeks and most people adapt well, but it is the appliance most likely to be noticeable in speech. Clear aligners cause a milder and shorter-lived change, since the plastic is thin and covers the palate only slightly. Conventional fixed braces on the outside of the teeth affect speech least of all, because they barely intrude on the space the tongue uses.

Does speech therapy help with a brace-related lisp?

It is rarely needed. A lisp caused by an appliance is a mechanical problem rather than a speech disorder, and it resolves as the tongue learns the new geometry, usually within two to four weeks. Reading aloud for ten or fifteen minutes a day speeds that adaptation considerably, and deliberately practising the sounds that give trouble helps more than avoiding them. A referral is worth discussing if a speech difficulty existed before treatment started, or if the change has not improved at all after a couple of months, which is unusual.

Will a palatal expander affect my child's speech?

Usually yes, temporarily. An expander occupies part of the palate that the tongue uses for s and sh sounds, so a lisp and slightly muffled speech for the first week or two are expected rather than a sign of a problem. Extra saliva is common at the same time. Children adapt quickly, generally faster than adults, and reading aloud at home helps. Speech that is still noticeably affected several weeks in is worth mentioning at the next visit, since it occasionally indicates the appliance needs a small adjustment.
