# SARPE: Surgical Palate Expansion for Adults in the UK

The upper jaw (maxilla) is formed by two halves joined at the mid-palatal suture. In children and teenagers this suture remains open and can be widened using a palatal expander appliance. In adults the suture fuses, making non-surgical expansion either ineffective or dangerous. Surgically assisted rapid palate expansion — SARPE, also called SARME — is the established solution for adults who need a wider upper arch.

## Why non-surgical expansion does not work in adults

In children and young teenagers, the mid-palatal suture is a cartilaginous growth zone that responds readily to the gentle forces of a removable or fixed palatal expander. The two halves of the palate separate, new bone fills in the gap, and the arch widens. By the mid to late teens the suture begins to interdigitate — the two bony edges become interlocked — and by the early to mid twenties it is typically fully fused in most adults. Applying expansion forces to a fused suture in an adult does not open it; instead, it tips the upper back teeth outward, thins the buccal bone, and risks gum recession and root exposure. For adults with a genuine skeletal transverse deficiency requiring significant expansion, surgery is therefore necessary.

## The SARPE procedure

SARPE is performed under general anaesthetic by an oral and maxillofacial surgeon (OMFS). The surgeon makes osteotomy cuts in the bone around the mid-palatal suture — typically along the lateral walls of the nasal cavity and through the pterygoid plates — to release the suture from the surrounding skeleton. A distractor device (the expander) is fitted to the palate at the same operation. This device has a central screw mechanism that is activated after a short healing period — usually five to seven days post-surgery — to begin separating the two halves of the palate. The device may be tooth-borne, bone-borne, or tooth and bone-borne depending on the patient's anatomy and the surgeon's preference.

## The expansion and retention protocol

Once activation begins, the patient or a carer turns the screw one or two quarter-turns per day, producing approximately 0.25 mm of expansion per turn. The typical expansion rate is around 0.5 to 1 mm per day, and the total expansion required is usually between 5 and 10 mm. Activation continues for approximately two to four weeks until the target width is achieved. The device is then left in place as a retainer for a further three to six months while new bone forms in the midline gap — a process called ossification. The temporary gap between the upper central incisor teeth (a diastema) that opens during expansion closes spontaneously as the teeth are later aligned with braces.

## Integration with orthodontic treatment

SARPE is rarely a standalone procedure. It is almost always part of a combined orthodontic and surgical treatment plan. Fixed braces or clear aligners are placed after the expansion phase to align the teeth and close any residual spaces. In some cases SARPE is combined with other orthognathic procedures if the patient also has a significant antero-posterior jaw discrepancy. Treatment is coordinated between the orthodontist and the oral and maxillofacial surgeon, and thorough pre-treatment planning — including CBCT imaging and study models — is essential.

## NHS vs private costs in the UK

SARPE may be available on the NHS where there is a significant functional or clinical indication, though access varies by region and many adult patients are treated privately. Private SARPE costs in the UK typically range from £5,000 to £12,000 or more depending on the complexity, the hospital fees, the surgeon's fees, and whether orthodontic treatment is included in the quotation. Patients should ensure they receive a full breakdown of surgical fees, anaesthetic fees, hospital costs, and orthodontic fees separately. Some private hospitals offer finance options for surgical procedures.

## MARPE: the minimally invasive alternative

Micro-implant-assisted rapid palate expansion (MARPE) uses four small titanium mini-screws placed through the palatal gingiva into the palatal bone to anchor a bone-borne expander — without surgical osteotomies. Because the force is applied directly to bone rather than through teeth, some degree of true suture opening can be achieved in younger adults (typically under 30) where the suture is incompletely fused. MARPE is performed under local anaesthetic as a relatively minor procedure compared to full SARPE. Success rates are variable and depend heavily on the degree of suture fusion, patient age, and expander design. CBCT imaging is typically used to assess suture morphology before deciding between MARPE and SARPE. For patients with a fully fused suture or older adults, SARPE remains the more predictable option.

### Frequently Asked Questions

At what age does the mid-palatal suture fuse?

The suture begins to interdigitate in mid-adolescence and is typically fully fused by the early to mid twenties, though there is individual variation. Some adults in their late teens may still have enough suture mobility for MARPE; most adults over 25 require SARPE for predictable expansion.

Is SARPE painful?

The surgery itself is carried out under general anaesthetic so there is no intraoperative pain. Post-operatively, patients experience swelling, bruising, and discomfort for one to two weeks, manageable with prescribed pain relief. The activation phase — turning the screw — is uncomfortable rather than painful for most patients, with a sensation of pressure across the nose and cheeks. A gap opening between the front teeth can feel strange but is expected and temporary.

How long does SARPE treatment take overall?

The surgical procedure itself takes one to two hours. After a week of healing, activation takes two to four weeks. The device is retained for a further three to six months. Combined with the subsequent orthodontic phase, total treatment from surgery to removal of braces is typically eighteen months to three years depending on the complexity of alignment required.

Will I need to stay in hospital after SARPE?

Most SARPE procedures are performed as day-case or one-night inpatient procedures. A hospital stay of one night is common, though some surgeons discharge patients the same day. You will need to be nil-by-mouth before the general anaesthetic and will require someone to accompany you home.

Can SARPE be done on the NHS?

SARPE may be available on the NHS for patients with a significant clinical indication — for example, a skeletal transverse discrepancy contributing to a crossbite, breathing difficulties, or as part of a combined orthognathic case. Access varies by region; in many areas there are long waiting lists or SARPE is not commissioned as a standalone procedure for adults. Referral to an NHS oral and maxillofacial surgery unit by your orthodontist is the usual route.

What is the gap between my front teeth during SARPE?

The temporary gap (diastema) between the upper central incisors during active expansion is a normal and expected finding. It appears because the two halves of the palate are being physically separated. Once activation is complete and the retention phase begins, the gap typically begins to close on its own as the stretched soft tissue recoils. Any remaining gap is subsequently closed with braces.

What happens if SARPE is not done and my palate is too narrow?

If a significant upper arch constriction is left untreated in an adult, the consequences depend on severity. Dental compensation (tipping the upper teeth outward) is unstable and carries a risk of gum recession. A posterior crossbite can cause asymmetric jaw function and contribute to TMJ symptoms. Severe constriction can affect the nasal airway. For patients with significant skeletal transverse discrepancy, SARPE or orthognathic surgery is usually the most stable long-term solution.
