# Orthodontic Treatment Plan

A good orthodontic treatment plan explains what will be treated, why, how it will be monitored, what it may cost, and what happens afterwards.

## What a thorough orthodontic treatment plan should include

A complete orthodontic treatment plan should document: the clinical diagnosis (what is being treated and why), proposed appliance type and reasons, estimated treatment duration and number of appointments, what records were taken and what they showed (X-rays, photographs, scans), risks specific to your case (root resorption risk, gum considerations, any complex movements), alternatives that were considered and why they were not recommended, total fee and itemised inclusions, retainer plan and whether retainers are included, and what happens if treatment changes mid-course. Anything less than this is incomplete.

## Diagnosis: the foundation of any treatment plan

A proper diagnosis requires: clinical examination of teeth, gums, and jaw; X-rays (OPG panoramic and lateral cephalometric at minimum); dental photographs; and bite assessment in three dimensions. Without X-rays, a clinician cannot assess root positions, bone levels, or whether any teeth are impacted — information that directly affects what treatment is safe. If a treatment plan is presented without any X-ray records, this is a significant warning sign. Diagnosis drives the plan; the plan should not be built before diagnosis.

## How to read and compare written quotes

Two treatment plans can have identical headline prices but include very different things. When comparing quotes, check specifically: Are records (X-rays, photographs, scans) included or charged separately? How many adjustment appointments are included? Are refinements included (for aligner plans) and how many? Are retainers included, and for how long? What is the repair policy for broken brackets or lost aligners? Is there a reviews period after treatment ends? Is finance available, and if so, what is the total payable (not just monthly)? Request itemised quotes in writing from every provider you compare.

## Red flags in orthodontic treatment plans

Be cautious of: a treatment plan presented without clinical records or X-rays; a fee quoted immediately at first consultation before any records are taken; no discussion of risks; no retention plan mentioned; high-pressure sales tactics or same-day commitment offers; very low prices that don't align with what is included; and no written documentation. A clinician who cannot explain why a particular appliance is recommended for your case has not performed an adequate diagnostic assessment.

## How treatment plans can change during treatment

Orthodontic treatment plans are forecasts based on the best available clinical information at the start. Plans legitimately change when: teeth move differently than predicted (biological variation), new X-ray findings emerge (unexpected root resorption, changed bone levels), a patient's oral health changes (new gum disease, tooth decay), additional refinements are needed for aligners, or a patient requests a change of appliance type. Any significant change to a treatment plan should be communicated to you in writing, with the reason for the change and any impact on cost or timeline explained.

## Before you sign: a checklist

Before signing any orthodontic treatment contract, confirm in writing: total treatment fee and payment terms; what records are included; number of included appointments; refinement policy (for aligners); retainer type, cost, and whether included; repair and replacement policy; what happens if treatment does not achieve the planned result; and the clinician's GDC registration number and specialist status. Ask for 24–48 hours to review before signing. A reputable practice will not pressure you to sign immediately.

### Frequently Asked Questions

What should an orthodontic treatment plan include?

A thorough treatment plan should include: diagnosis and clinical findings, proposed appliance and reasons, estimated duration and number of appointments, records taken (X-rays, photos, scans), risks specific to your case, alternatives considered, total itemised fee, retainer plan, and what happens if treatment changes. A plan presented without X-ray records or with no risk discussion is incomplete. Always request a written copy before signing.

Should a treatment plan include retainers?

It should explain the retention plan clearly — which type of retainer will be provided, whether it is included in the fee or charged separately, the replacement policy, and how long the review period lasts after treatment. Retention is part of orthodontic treatment, not an optional add-on. If a treatment plan does not mention retention at all, ask specifically before signing.

Can a treatment plan change after I start?

Yes — plans change legitimately when teeth move differently than predicted, new clinical findings emerge, or patient circumstances change. Any significant change should be communicated in writing with the reason explained and any cost or timeline impact made clear. You have the right to understand why a change is being made and to seek a second opinion before agreeing to a changed plan.

Should I get a written quote before starting orthodontic treatment?

Always. A verbal quote provides no protection if a dispute arises later. A written itemised quote lets you compare providers accurately, understand what is included, review the total payable (not just the monthly payment), and identify any significant omissions. In the UK, patients are entitled to a written treatment plan and cost estimate under GDC standards. If a practice refuses to provide one, this is a red flag.

How do I compare two orthodontic treatment plans?

Compare line by line, not just the total. Look at: records included (X-rays, photos, scans); number of appointments; refinements included (for aligners); retainer type and whether included; repair policy; reviews period; finance terms (total payable, not just monthly). A £3,500 plan that includes everything may be better value than a £2,800 plan that charges separately for records, retainers, and refinements.

What happens to my treatment plan if I move house mid-treatment?

Ask for this to be addressed in the plan before you start, particularly if you are a student or expect to relocate. Transferring an in-progress case is possible but not automatic: the receiving practice must be willing to take it on, familiar with the appliance system in use, and able to obtain your full records. Fees for taking over a partly completed case vary and are set by the new practice. Check what your original practice refunds for treatment not yet delivered, since policies differ substantially.
