# Orthodontic Space Closure UK

Orthodontic space planning decides whether gaps should be closed, redistributed, or deliberately opened for implants, bridges, or bonding.

## Close or open spaces — the central planning decision

Closing every gap is not automatically the best outcome. The decision depends on how much crowding there is elsewhere in the arch, the size and shape of the remaining teeth, the bite relationship between the arches, the facial profile, gum health, and what restorative work might be planned later. Closing space moves teeth forward or backward through bone, which changes lip support and the position of the front teeth — that is a benefit in a protrusive case and a drawback in a flat one. Opening or maintaining space preserves the tooth positions but commits the patient to a restoration to fill the gap. Your orthodontist should explain which approach they are recommending and why, ideally with reference to your records rather than in the abstract.

## Extraction spaces and how they close

When teeth are removed to relieve crowding or reduce protrusion — most commonly first or second premolars — the resulting space is closed gradually with braces or aligners. Space typically closes at roughly one millimetre per month, so a full premolar space of seven to eight millimetres takes the best part of a year to close on its own. The clinical difficulty is not closing the space but controlling how it closes: whether the front teeth come back, the back teeth come forward, or both meet in the middle. Roots must be kept upright as teeth translate, because tipping teeth into a space produces a gap that looks closed at the crown while the roots remain apart, which is unstable and shows on the final X-ray.

## Anchorage — controlling which teeth move

Anchorage is the resistance to unwanted tooth movement, and it is the single most important planning variable in space closure. Every force applied to pull a tooth forward pulls something else backward in equal measure. Where the front teeth must be retracted and the molars held still, the orthodontist reinforces anchorage using a transpalatal arch, a Nance button, headgear, or increasingly a temporary anchorage device — a small titanium mini-screw placed in the bone under local anaesthetic and removed at the end of treatment. Mini-screws have made maximum anchorage cases considerably more predictable in UK practice and avoid the compliance problems of headgear, though they add cost and carry a small risk of loosening.

## Missing teeth: hypodontia and space management

Hypodontia — developmentally absent teeth — affects roughly four to six per cent of people in the permanent dentition once wisdom teeth are excluded, and the teeth most often missing are the upper lateral incisors and the lower second premolars. Where an upper lateral incisor is absent, there are two established routes. Canine substitution moves the canine forward into the lateral incisor position and reshapes it with composite or a veneer, avoiding any implant. Space opening keeps or creates the correct width for an implant or a resin-bonded bridge. Which is preferable depends on the canine's colour and shape, the bite, the gum line, and the patient's age. Complex hypodontia cases are usually managed by a joint clinic involving an orthodontist and a restorative dentist.

## Why implants change the timing

A dental implant fuses directly to bone and does not move with subsequent growth, so it behaves like an ankylosed tooth. Placing one before facial growth is complete leaves the implant crown progressively lower than the neighbouring teeth as the face continues to develop. For this reason implants are normally deferred until growth has finished — commonly around eighteen for females and later, often into the early twenties, for males, confirmed where necessary with serial records rather than age alone. Because orthodontic treatment usually finishes years earlier, the space must be held in the interim. A resin-bonded (Maryland) bridge or a tooth added to a removable retainer is the usual solution, and it is often the definitive one if the patient is happy with it.

## Diastema closure and why gaps reopen

A midline gap between the upper central incisors can close readily with braces or aligners, but it is one of the most relapse-prone movements in orthodontics. Before closing it, the cause needs identifying: a prominent labial frenum, a buried supernumerary tooth (mesiodens) sitting between the roots, generalised spacing from small teeth relative to arch size, or a tongue habit. A supernumerary must be removed and a frenum may need a frenectomy, or the space simply reopens. Long-term stability normally depends on a bonded wire retainer behind the front teeth, and where the teeth themselves are undersized, composite bonding to build them to proportionate width is what actually resolves the space rather than moving teeth alone.

### Frequently Asked Questions

Can braces close extraction gaps completely?

Yes. Fixed braces close premolar extraction spaces routinely and can close them fully, but it takes time — space closes at roughly a millimetre a month, so a full premolar space needs the better part of a year. The clinical priority is keeping the roots upright as the crowns come together, because tipping teeth into a space leaves the roots apart even though the gap looks closed. Your orthodontist checks this on a progress X-ray before finishing.

Should a missing tooth space be closed or opened?

It depends on which tooth is missing, how much crowding there is elsewhere, the bite, the shape and colour of the neighbouring teeth, and your age. Closing the space avoids any implant, bridge, or lifelong restoration and is often preferable where there is crowding to relieve anyway. Opening or keeping the space gives a more conventional tooth arrangement but commits you to a restoration. For a missing upper lateral incisor both routes are well established, and the choice is usually made jointly by an orthodontist and a restorative dentist.

Can clear aligners close gaps?

Often yes. Aligners handle generalised spacing and small to moderate gaps predictably, and closing spaces is one of the movements they do well because it is largely tipping and translation of crowns. Large extraction space closure is harder: it needs bodily movement of teeth through several millimetres of bone with good root control, which is where fixed braces still have the advantage. Many aligner plans for extraction cases use attachments and sometimes mini-screws, and some are better treated with braces from the outset.

What is anchorage and why does my orthodontist keep mentioning it?

Anchorage is the resistance to unwanted tooth movement. Any force used to pull one tooth forward pulls something else backward by the same amount, so the plan has to specify which teeth are allowed to move and which must be held still. Where the front teeth need retracting and the back teeth must not drift forward, the orthodontist reinforces anchorage with a palatal arch, a Nance button, headgear, or a temporary anchorage device — a small mini-screw placed in bone under local anaesthetic and removed at the end.

Why can't I have an implant as soon as my braces come off?

Because an implant fuses to bone and then stays put while the rest of your face keeps growing. Placed too early, the implant crown ends up sitting progressively lower than the teeth on either side as growth continues, which is difficult to correct afterwards. Implants are therefore normally deferred until growth is complete — commonly around eighteen for women and often into the early twenties for men. In the meantime the space is held with a resin-bonded bridge or a tooth added to your retainer.

Why did my midline gap come back after treatment?

Diastema closure is one of the most relapse-prone movements in orthodontics, and a gap that reopens usually means the underlying cause was not addressed. Common causes are a prominent labial frenum, a buried extra tooth sitting between the roots, or front teeth that are simply narrower than the space available. Long-term stability normally requires a bonded wire retainer behind the front teeth, and where the teeth are undersized, composite bonding to build them to proportionate width is what actually resolves it.

Do mini-screws hurt, and are they permanent?

They are temporary and removed at the end of treatment. Placement takes a few minutes under local anaesthetic and most patients describe pressure rather than pain; the area can feel tender for a day or two afterwards, similar to having a brace tightened. Removal usually needs no anaesthetic at all and the gum heals over within a couple of weeks. The main practical issues are keeping the area clean and the small chance that a screw loosens and needs re-siting.
