Getting Started

# Orthodontic Referral Letter UK

An orthodontic referral letter summarises why assessment is needed and helps the orthodontist prioritise the next step.

## What a referral letter includes

A referral usually includes patient details, the dental concern, relevant medical history, oral health status, suspected IOTN need, and any X-rays or notes already available. For NHS care, the referral must show clinical need rather than cosmetic preference alone.

## NHS referrals

NHS orthodontic referral is usually made by a dentist for under-18s who may meet IOTN eligibility. The orthodontic provider then assesses the case, confirms IOTN, and places eligible patients on the waiting list or declines NHS treatment if the threshold is not met.

## Private referrals

Private orthodontists often accept self-referrals, so a letter may not be required. A dentist referral can still be helpful if there are gum, decay, missing tooth, or restorative issues that affect planning.

## What happens after referral

You may receive an assessment appointment, wait-list confirmation, request for more information, or a rejection if criteria are not met. Waiting times vary by area and provider capacity.

## What to bring

Bring any referral details, X-rays if provided, your dental history, retainer history if relevant, and a list of questions. Photos from home can also help you explain what bothers you.

### Frequently Asked Questions

Do I need a referral for private braces?

Usually not. Most private orthodontic practices in the UK accept direct enquiries and will book you in for an assessment without any letter from a dentist. A referral is still worth having if your dentist has identified something that affects planning — active gum disease, untreated decay, a missing or buried tooth, or existing crowns and veneers — because it saves the orthodontist repeating investigations and avoids a treatment plan being revised after it has been quoted.

Who writes an NHS orthodontic referral?

Normally your general dental practitioner, who sees your teeth at routine check-ups and is best placed to judge when assessment is warranted. They record the clinical concern, note your likely IOTN grade, and send the referral to an NHS orthodontic provider — a hospital department, a community dental service, or an NHS-contracted practice. A referral can also come from a school dental service or, less commonly, from another specialist already involved in your care.

Does a referral mean I qualify for NHS braces?

No. A referral means your dentist thinks assessment is warranted, not that treatment has been approved. The orthodontic provider carries out its own examination and scores the case formally against the NHS England IOTN threshold: a Dental Health Component grade of 4 or 5, or a grade 3 case where the Aesthetic Component scores 6 or more. Cases that fall below that threshold are declined for NHS treatment, and the practice should tell you why and what your private options are.

Can I see the referral letter?

Yes. It forms part of your dental record, so you can ask your dentist or the receiving orthodontic practice for a copy, and under UK data protection law a Subject Access Request cannot normally be charged for. Reading it is genuinely useful — it tells you what clinical problem is being referred, what IOTN grade your dentist has estimated, and whether any X-rays or notes have been sent, so you can prepare questions for the assessment.

How long will I wait after an NHS referral?

Waiting times vary considerably by area and provider capacity, and run to four years or more in high-demand parts of England before treatment starts, against around six months in lower-demand areas. There are often two separate waits — one for the initial assessment appointment and a second, usually longer one between being accepted and appliances actually being fitted. If your child is close to eighteen it is worth asking the provider directly how the wait affects eligibility, since NHS orthodontics is largely an under-18 service.

What happens if my referral is rejected?

Rejection usually means the case did not meet the IOTN threshold, or that key information was missing from the letter. Ask which of the two applies, because they lead to different next steps. If information was missing, your dentist can re-refer with the additional records. If the case genuinely scored below the threshold, NHS treatment is not available, but the concern may still be treatable privately — and a case that is borderline now can score differently once remaining adult teeth have erupted, so a re-assessment later is sometimes reasonable.

Can I be referred to a specific orthodontist?

You can express a preference and many dentists will accommodate it, but NHS referrals are directed to commissioned providers and some areas operate a central referral management system that allocates cases rather than letting patients choose. For private treatment you have a free choice — you can approach any practice directly. If you are choosing, check the GDC register to confirm whether the clinician is a registered specialist orthodontist rather than a general dentist providing orthodontics.
