# NHS Orthodontic Waiting List UK: How Long and What to Do

NHS orthodontic waiting lists run to four years or more in many parts of England, though around six months in lower-demand areas. This guide explains who qualifies, how long you can realistically expect to wait, and what private and online alternatives are available.

## How Long Are NHS Orthodontic Waiting Lists?

NHS orthodontic waiting times vary significantly by region but have lengthened considerably since 2020.

In many parts of England, Wales, and Scotland, waits of 2–4 years are common for patients who qualify. In some areas, waits can extend to 5 years or longer.

Adults are not routinely eligible for NHS orthodontic treatment in the UK.

## Who Qualifies for NHS Orthodontic Treatment?

NHS orthodontic treatment in the UK is primarily available to children and young people under 18 whose teeth meet a clinical threshold.

Eligibility is assessed using the Index of Orthodontic Treatment Need, a scoring system that grades the severity of dental misalignment from 1 to 5.

## What Are the IOTN Grades for NHS Eligibility?

### Grades 1–2

No or little treatment need. Mild irregularities are not usually funded by the NHS.

Not Eligible

### Grade 3

Moderate treatment need. These are borderline cases and may depend on local policy.

Sometimes

### Grade 4

Great treatment need, usually involving significant misalignment or bite issues.

### Grade 5

Very great treatment need, including severe misalignment or complex clinical cases.

## How Does the NHS Referral and Waiting List Work?

Your general dentist makes the referral, usually in the early teenage years, to a local orthodontic assessment service or specialist practice. At the assessment the orthodontist examines the teeth, bite and jaw growth, may take photographs, X-rays or scans, and scores the case on the IOTN. Cases that meet the threshold join the treatment waiting list; cases that do not are discharged back to the dentist with advice about private options.

Waits differ so much by area because each Integrated Care Board commissions a fixed number of NHS orthodontic cases a year, set against local demand, specialist workforce and the number of NHS contracts. Within the list, the most severe cases (impacted teeth, large overjets, bites that affect chewing) are usually prioritised, and patients with the same IOTN grade are then generally seen in date order.

While you wait, keep up routine dental check-ups and good brushing, since active decay or gum disease will delay the start of braces. Tell the referring dentist about any new pain, trauma or change in bite, as it may change the IOTN score, and keep your contact details up to date so an appointment offer is not missed.

## What Are the Alternatives to Waiting?

### Private Fixed Braces

Metal or ceramic braces accessed privately without waiting. Suitable for a wide range of case types.

### Clear Aligners

Removable transparent trays such as Invisalign or other aligner systems. Usually suited to mild to moderate cases.

### Online Quote First

Upload photos and receive clinician-reviewed treatment options and pricing before visiting a practice.

### Payment Plans

Many private practices offer 0% or low-interest plans to spread the cost across treatment.

## Should I Wait for NHS Treatment or Go Private?

If your child qualifies for NHS treatment at IOTN grade 4 or 5, it is usually worth joining the waiting list even if you also explore private options.

However, for adults, patients who do not meet the IOTN threshold, or families who need treatment to begin quickly, private orthodontics is often the more practical route.

An online quote gives you a realistic picture of what private treatment could cost for your specific case before you commit.

### FAQs

How long is the NHS orthodontic waiting list?

NHS orthodontic waiting lists vary widely by region: around six months in lower-demand areas, but four years or more in many parts of England, and a 2023-24 audit of all 42 integrated care boards documented a longest wait of 6.6 years. Only one board met the 18-week standard. Waits vary widely by region and by individual practice, because NHS orthodontic care is delivered under contracts with a fixed number of cases each year. Your dentist or the orthodontic practice holding the referral can give a realistic local estimate. Because NHS treatment normally has to begin before a patient's 18th birthday, it is worth asking about the expected wait early, particularly for children referred at 12 or 13.

Can adults get NHS braces in the UK?

Adults are essentially never eligible for NHS orthodontic treatment in the UK. NHS orthodontics is funded only for under-18s who meet the clinical threshold — IOTN dental health component 4 or 5, or component 3 with an aesthetic component of 6 or above. Once you turn 18 you fall outside that funding however crowded or misaligned your teeth are. The narrow exceptions are hospital-based: severe functional problems, cleft lip and palate, and orthodontics carried out as part of orthognathic jaw surgery, all of which are consultant-led. In practice, almost all adult orthodontic treatment in the UK is paid for privately.

What is IOTN and how is it calculated?

IOTN stands for the Index of Orthodontic Treatment Need, the scoring system NHS orthodontists in the UK use to decide whether a case qualifies for funded treatment. It has two parts. The dental health component grades the clinical severity of the problem from 1 (no need) to 5 (very great need), looking at features such as overjet size, impacted or missing teeth, crossbites and the degree of crowding. The aesthetic component compares the appearance of the front teeth against a ten-photograph scale. A patient qualifies at dental health component 4 or 5, or at component 3 with an aesthetic score of 6 or above.

What happens if my child ages out of the waiting list?

NHS orthodontic treatment normally has to start before a patient's 18th birthday, so a child who turns 18 while still waiting will usually lose access to NHS funding and would have to pay privately from that point. Practices are aware of this and often take a patient's age into account when managing the list, but there is no guarantee. If your child was referred at 15 or 16 and the local wait is several years, ask the referring dentist or the orthodontic practice directly what a realistic start date looks like, and weigh up whether starting privately is safer than risking the deadline.

How much does private orthodontic treatment cost compared to NHS?

NHS orthodontic treatment is free for under-18 patients who meet the IOTN threshold, so the comparison is between nothing and the full private fee. Private treatment in the UK typically costs £1,500–£5,500, and where you sit in that range is set mainly by the appliance and how complex the case is: metal fixed braces sit at the lower end, ceramic and clear aligner systems higher, and lingual braces higher again. The quoted fee normally covers records, the appliance, routine adjustment appointments, removal and a first set of retainers, but confirm exactly what is included before signing, as replacement retainers and refinement aligners are often charged separately.

Can my child start private braces while waiting for NHS treatment?

Yes. Being on the NHS waiting list does not prevent you from starting private treatment. Many families choose private braces when the NHS wait is too long, particularly if the child is at risk of aging off the list before treatment starts. Policies differ between providers and areas: in most cases you are removed from the NHS list once private treatment for the same problem begins, so check with both the NHS provider and the private clinician before starting.

Does a referral guarantee NHS braces?

No. A referral from your dentist only guarantees an assessment appointment with an orthodontist, not funded treatment. At that appointment the orthodontist scores the case on the Index of Orthodontic Treatment Need and applies the local commissioning threshold, generally dental health grade 4 or 5, or grade 3 with an aesthetic score of 6 or above. Children whose cases fall below the threshold are discharged back to their dentist, usually with advice that private treatment is the only route if the family still wants the teeth straightened.

What happens at the first NHS orthodontic assessment appointment?

The orthodontist examines the teeth, bite and stage of jaw development, and may take photographs, X-rays or a digital scan. They then calculate the IOTN score and explain whether the case qualifies for NHS treatment. If it does, your child is added to the treatment waiting list, and the orthodontist may say whether treatment is best started now or once more adult teeth have come through. If it does not qualify, you are discharged back to your general dentist, usually with information about private options.

Can I speed up an NHS orthodontic waiting list?

Usually not, because places are allocated on clinical need and then date order rather than on request. The main exception is a genuine change in circumstances, such as new pain, dental trauma or a worsening bite, which may raise the IOTN score, so report these to the referring dentist promptly. It also helps to keep contact details up to date and ask the practice whether it offers short-notice or cancellation appointments, since patients who can attend at short notice are sometimes started sooner.
