# Getting NHS Braces as an Adult

NHS orthodontic treatment is primarily for patients under 18. Most adults are not eligible — but there are rare exceptions. Here is everything you need to know, plus the fastest private routes if you don't qualify.

## Can adults get NHS braces in England?

In almost all cases, no. NHS orthodontic treatment in England is funded for patients under the age of 18 who meet the clinical threshold set by the Index of Orthodontic Treatment Need (IOTN). Once you turn 18, eligibility for NHS-funded orthodontics effectively ends — regardless of how crooked your teeth are, how much you want treatment, or whether your family's NHS dental practice will refer you.

This is not a matter of waiting times or regional variation — it is NHS policy. Adult cosmetic orthodontics is explicitly excluded from NHS commissioning. So even if you have a very high IOTN score as an adult, the NHS will not fund your braces.

For how IOTN scoring works at any age, and what to do if you are not eligible, see our guide to [who qualifies for NHS braces](https://virtualconsultant.online/about/do-i-qualify-for-nhs-treatment).

## When can adults access NHS orthodontics?

There are two narrow clinical circumstances in which adults may be entitled to NHS orthodontic treatment:

**Orthognathic surgery (jaw surgery):** If you require corrective jaw surgery — for example, to correct a severe skeletal class II or III relationship, or to treat obstructive sleep apnoea with a surgical solution — orthodontic preparation and finishing treatment before and after surgery is typically funded on the NHS alongside the surgical procedure. This is a complex, multi-year pathway and is not available simply because you want straighter teeth.

**Cleft lip and palate:** Patients born with cleft lip and/or palate who are managed through an NHS cleft team may receive orthodontic treatment as part of their ongoing multidisciplinary care into adulthood.

**Certain medically necessary cases:** In rare circumstances where orthodontics is part of a treatment plan for a recognised medical condition — for example, hypodontia managed through a dental hospital — NHS funding may apply. These are exceptional cases, assessed individually.

## What about patients who started NHS treatment as a child?

If you started an NHS orthodontic course of treatment before the age of 18, that course of treatment is generally completed by the NHS even if you turn 18 during treatment. However, if you were on a waiting list and had not yet started treatment when you turned 18, your eligibility may lapse — you would need to discuss this with the referring dentist or NHS orthodontic service.

## What are the private options for adults?

Private orthodontic treatment is available to adults of any age, without any clinical eligibility threshold. The range of treatment options available privately is typically broader than what is offered on the NHS, and waiting times are negligible compared to NHS lists.

| Treatment | Typical cost (UK 2026) | Best for |
| --- | --- | --- |
| **Metal fixed braces** | £1,500 – £3,500 | Most orthodontic problems; most cost-effective option |
| **Ceramic braces** | £2,000 – £4,500 | Adults wanting less visible fixed braces |
| **Clear aligners (e.g. Invisalign)** | £2,500 – £6,500 | Adults prioritising aesthetics and removability |
| **Lingual braces** | £4,000 – £8,000+ | Completely hidden treatment; complex cases |

Most practices offer 0% interest payment plans, spreading treatment costs over 12–24 months. Getting a photo-based quote online is the fastest way to understand your likely cost before committing to a consultation.

### Frequently Asked Questions

Can adults get NHS braces in the UK?

In almost all cases, no. NHS orthodontic treatment is funded for patients under 18 who meet the IOTN clinical threshold. Once you are 18, you are not eligible for NHS orthodontics for cosmetic or general dental alignment reasons. The only adults who may qualify are those requiring orthodontics as part of NHS-funded jaw surgery (orthognathic surgery) or ongoing cleft palate care.

Is there an age limit for NHS braces?

Yes. NHS orthodontics is primarily commissioned for patients under 18 in England. There is no formal blanket cut-off to the day, but NHS commissioning guidance makes clear that adult cosmetic orthodontics falls outside NHS provision. Patients who started treatment before 18 typically have their course completed.

Can I get free braces as an adult in the UK?

Free NHS braces for adults are not available under normal circumstances. If you need orthodontics as part of orthognathic (jaw) surgery funded on the NHS, the orthodontic component is covered. Otherwise, adult braces are a private expense. Many practices offer 0% interest payment plans to spread the cost.

What is the cheapest way to get braces as an adult?

The most affordable route is typically metal fixed braces with a private orthodontist, costing £1,500–£3,500 depending on complexity and location. Some practices offer payment plans. Getting photo-based quotes from multiple providers before committing to a paid consultation helps compare costs efficiently.

Can I get Invisalign on the NHS as an adult?

Invisalign is not available on the NHS, for adults or for children. NHS orthodontic contracts commission fixed metal braces, which are the appliance used to treat the crowding, protrusion and bite problems that qualify for NHS care, and clear aligner systems fall outside what is funded. Adults are in any case essentially never eligible for NHS orthodontics, the main exception being treatment that forms part of an NHS orthognathic surgery pathway — and even then the appliance would be a fixed brace. Invisalign and comparable systems are therefore a private option, priced by the practice and often spread over monthly instalments.

What happens if I was on the NHS waiting list and turned 18?

If you were on an NHS orthodontic waiting list when you turned 18 and had not yet started treatment, your eligibility may lapse. This varies by commissioner and practice. It is important to ask your referring dentist or the NHS orthodontic service before your 18th birthday to understand what will happen to your position on the list.
