# Crowded Lower Teeth UK

Crowding of the lower front teeth is one of the most common orthodontic concerns in the UK. Whether you have mild overlapping or severe bunching, treatment options range from IPR and aligners to premolar extractions and fixed braces — and long-term retention is essential to keep the result stable.

## Why does lower arch crowding happen?

The lower arch — particularly the lower front teeth — is one of the most crowding-prone areas in the mouth. The lower jaw tends to be slightly narrower than the upper, giving less space for the teeth. Lower incisors typically erupt later than upper incisors, sometimes finding little room remaining. Late mesial drift of posterior teeth and forward pressure on the front teeth is a well-documented phenomenon that continues well into adulthood. Genetics plays the largest role: tooth size relative to jaw size is largely inherited.

## How is lower crowding classified by severity?

Orthodontists classify lower crowding by the space discrepancy — the difference between available space and the space the teeth require. Mild crowding is 0 to 4 mm, moderate is 4 to 8 mm, and severe is above 8 mm. Severity guides the treatment approach: mild crowding can often be addressed without extractions; moderate crowding sits in a grey zone; severe crowding almost always requires extractions or significant arch development for a stable result.

## What is IPR and when is it used for lower crowding?

Interproximal reduction (IPR) is the careful removal of a small amount of enamel from the sides of teeth to create space. IPR is commonly used for mild lower crowding, typically creating 0.25 to 0.5 mm per contact point. Research consistently shows that IPR performed within established limits does not meaningfully increase decay risk or cause sensitivity long-term. It can only address mild crowding — attempting IPR for moderate or severe crowding would require removal of clinically inappropriate amounts of enamel.

## Are extractions needed for severe lower crowding?

For moderate to severe lower crowding, extraction of lower premolars is the most predictable way to create sufficient space. Premolars sit in the middle of the arch and their removal gives balanced space both to relieve front crowding and close the gap at the back. Extraction-based treatment has a long evidence base and produces stable results when followed by appropriate retention. Concerns about gaps or facial changes are unfounded — well-planned space closure leaves no visible gaps, and lower premolar extractions have minimal effect on facial appearance.

## Why is lower arch expansion usually not recommended?

Arch expansion — widening the jaw — is much more limited and less stable in the lower arch than the upper. The lower jaw is bounded by the tongue on the inside and the lips and cheeks on the outside, and moving teeth outside these soft tissue boundaries leads to instability and relapse. Most orthodontic guidelines recommend against significant lower arch expansion as a means of creating space. Small amounts of decompression may be possible, but true arch widening is generally not recommended in the UK.

## What role does retention play after lower crowding treatment?

Lower incisor crowding has a very strong tendency to recur after orthodontic treatment if retention is inadequate. A bonded (fixed) lower retainer — a thin wire glued to the backs of the lower front teeth — is the most reliable way to prevent relapse. Most UK specialist orthodontists now place these as standard. The retainer must be maintained indefinitely, and removable retainer wear should also be continued, particularly in the early years after treatment.

### Frequently Asked Questions

Can lower teeth crowding be fixed without extractions?

Mild lower crowding — typically up to about 4 mm of space discrepancy — can usually be resolved without extractions using IPR to create small amounts of space, combined with alignment with braces or aligners. Moderate crowding sits in a grey zone where the approach depends on the individual case. Severe crowding of more than 8 mm almost always requires lower premolar extractions to create sufficient space for a stable, well-aligned result. Expansion of the lower arch is generally not recommended as a space-gaining strategy as it tends to be unstable.

Does IPR damage teeth?

When performed within established clinical limits, IPR does not meaningfully damage teeth. Removing up to 0.25 mm from each tooth side in the lower anterior region leaves well over 80% of the enamel intact. Long-term research shows no significant increase in sensitivity or decay risk compared to untreated teeth. The enamel surfaces are polished after IPR, and in patients with good oral hygiene and normal fluoride exposure the surfaces remineralise rapidly.

Can aligners fix crowded lower teeth?

Yes, aligners can effectively treat mild to moderate lower crowding, particularly when combined with IPR to create space. Most clear aligner systems have some limitations with certain lower incisor movements — particularly bodily tipping and significant rotations — compared to fixed braces. For mild crowding this is rarely a limiting factor, but for more complex cases fixed braces may provide better control. Extraction cases requiring significant space closure are generally treated more reliably with fixed braces.

Will my teeth stay straight after lower crowding treatment?

Teeth will stay straight if adequate retention is maintained. A bonded lower retainer is the most effective form of retention for lower crowding and should be maintained indefinitely. Wearing a removable retainer at night provides additional security. Without retention, lower incisor crowding is very likely to recur over years — this is a well-documented biological tendency, not a failure of treatment.

Is lower crowding treated on the NHS?

Children under 18 with lower crowding that scores a DHC grade of 3.6 or above on the IOTN scale are eligible for NHS orthodontic treatment. Moderate to severe lower crowding typically scores at this level or above. Mild crowding may not reach the NHS threshold and would need to be treated privately if desired. Adults do not generally qualify for NHS orthodontics unless they meet specific medical exception criteria.

What is the realistic cost of treating lower crowding privately in the UK?

Private orthodontic treatment for lower crowding typically costs between £2,500 and £5,000 depending on the complexity of the case, whether extractions are needed, and the practice location. Simple aligner treatment for mild crowding using IPR may fall towards the lower end of this range. Cases involving extractions and full fixed braces are usually priced towards the upper end. Your orthodontist should provide a written cost plan before treatment begins.
