# Clear Aligner Refinements

Clear aligner refinements are additional planning stages used when teeth need further movement after the initial aligner sequence.

## What clear aligner refinements are

A refinement is an additional set of aligners prescribed after the initial aligner series has been completed or when teeth are not tracking as planned mid-treatment. After completing all aligners in an initial series, your clinician takes a new scan of your actual tooth positions and compares them to where the ClinCheck (treatment plan) predicted they should be. Any gap between actual and planned position is modelled into a new series of refinement aligners to finish the job. Refinements are a standard part of comprehensive aligner treatment — they are not a failure of treatment.

## Why teeth may not track as planned

Clear aligner 'tracking' refers to how well teeth follow the planned movements in each aligner. Non-tracking is common and has several causes: insufficient aligner wear time (under 20–22 hours/day), attachments that have come off or are ineffective, teeth that move slower than predicted (biological variation), complex rotation or root movements that aligners handle less predictably, and patient non-compliance with elastics. If aligners are consistently not tracking after multiple trays, your clinician may pause, reassess, and prescribe an early refinement.

## How many refinements are typical

For Invisalign Comprehensive: multiple refinements are included in the original fee with no limit (under the 'unlimited refinements' protocol). For most other systems (Spark, SureSmile, ClearCorrect) comprehensive packages typically include at least one or two refinement rounds. Short-course packages (Invisalign Lite, most DTC systems) typically include no refinements or only one. A typical comprehensive Invisalign case may use 1–3 refinement rounds. Unusually complex or non-compliant cases may use more.

## What happens during a refinement

At a refinement appointment, your clinician takes new intraoral scans of your current tooth positions. These are submitted to the aligner lab (Invisalign, Spark, etc.), which produces a new ClinCheck showing the movements still needed. You and your clinician review and approve the new plan. New aligners are fabricated and shipped — this typically takes 2–4 weeks. You continue wearing your existing retainer or final aligner from the previous series until the new aligners arrive.

## Refinements and the impact on total treatment time

Each refinement round adds time: 2–4 weeks of fabrication plus however many weeks of wearing the new series. A single refinement round typically adds 2–5 months to treatment. Multiple refinements can extend treatment by 6–12+ months beyond the original estimate. This is why strong compliance from the start — consistent 22-hour wear, keeping all appointments, maintaining attachments — significantly reduces the likelihood and number of refinements needed.

## What to check in your treatment contract about refinements

Before signing for clear aligner treatment, ask specifically: How many refinements are included in the fee? Is there a time limit on included refinements (some practices include refinements only within 24 months of starting)? What is charged if additional refinements are needed beyond what is included? What triggers the clinician to prescribe a refinement vs progressing to retainers? When will retainers be made — after the final refinement only? Getting clear written answers to these questions prevents surprise costs.

### Frequently Asked Questions

Are refinements a sign that treatment has gone wrong?

No — refinements are a normal and expected part of comprehensive aligner treatment. Even well-planned cases with excellent compliance often need a refinement to finish detailing the bite or close small residual gaps. Invisalign Comprehensive includes unlimited refinements specifically because they are anticipated. A refinement is only a concern if it is needed repeatedly due to non-compliance, persistent non-tracking, or poor clinical planning.

Do refinements cost extra?

It depends entirely on your treatment contract. Invisalign Comprehensive includes unlimited refinements within the active treatment period at no additional cost. Short-course packages (Invisalign Lite, many DTC systems) typically do not include refinements — each round may cost £300–£700 extra. Other brand comprehensive packages vary. Always ask for the refinement policy in writing before starting treatment — this is one of the most important financial questions to ask.

How long do refinements add to treatment?

Each refinement round typically adds 2–5 months: 2–4 weeks for new aligner fabrication, plus the wear time for the new series (which may be 10–30 additional aligners depending on what movements remain). A single refinement adds roughly 2–5 months. Multiple refinements can add 6–12+ months. Strong compliance from the start — wearing aligners 22 hours per day, keeping all appointments, protecting attachments — is the most effective way to reduce refinements needed.

When can retainers be made?

Retainers are made once the clinician confirms that the final tooth position has been achieved and is accepted as the outcome. This is typically after the last refinement series is complete, not after the first aligner series. Making retainers before refinements are finished would set the retention to the wrong tooth position. If you are offered retainers before a refinement discussion has taken place, ask your clinician whether the clinical outcome is truly finalised.

What if my aligners stop fitting during a refinement?

If refinement aligners don't fit correctly, stop wearing the non-fitting aligner and contact your clinician. Forcing a misfit aligner can move teeth incorrectly. Your clinician will take a new scan to assess what has happened. This may require another new refinement series. Do not skip to a later tray in the series — this is a common patient mistake that worsens tracking problems.

Can I request a refinement if I am unhappy with my current result?

Yes — if treatment has finished but you are not satisfied with the final position of specific teeth, you can request a refinement assessment. Whether this is included in your contract depends on the treatment package. Invisalign Comprehensive includes refinements within the active treatment period, so if you raise the concern before your clinician declares treatment complete, a further refinement scan is usually possible at no additional cost. After treatment is declared complete and retainers have been issued, any further treatment is typically a new course with new charges. Raise concerns about tooth positions before your debond appointment, not after.

What is the difference between a refinement and starting treatment again?

A refinement continues the current treatment from wherever the teeth are now — using a new scan to plan the remaining movements. Starting treatment again means going back to the beginning: new records, a new treatment plan covering the full correction, and a new fee. Refinements within a comprehensive package are extensions of the original treatment, not restarts. Starting again might be necessary if the original treatment was fundamentally incorrect, if a very long time has elapsed and teeth have moved significantly, or if a patient changes provider and the new clinician cannot access the original case files.
