# Class III camouflage orthodontics

Mild to moderate underbites can sometimes be improved without surgery by moving the teeth into a better bite, known as camouflage, but it is not suitable when the jaw discrepancy is large. Learn the limits, the gum risks and when surgery is more appropriate.

## What Class III means

Class III describes a bite where the lower teeth or jaw sit too far forward relative to the upper. It may appear as an edge-to-edge bite, anterior crossbite or visible underbite.

## What camouflage tries to do

Camouflage improves the tooth relationship without jaw surgery. It may procline upper incisors, retrocline lower incisors, use Class III elastics, create space, or coordinate arches so the bite functions better.

## Who is a good candidate

Best candidates have mild to moderate discrepancy, acceptable facial profile, healthy gums and teeth that can be moved safely within bone support. Adults need especially careful periodontal assessment.

## Limits and gum risks

Too much incisor tipping can create recession risk, unstable results or an unnatural tooth angle. Camouflage cannot correct a major skeletal jaw-size discrepancy or facial imbalance.

## When surgery is more appropriate

Surgery is more likely when the underbite is severe, chewing is compromised, the profile concern is skeletal, or dental compensation would push teeth beyond healthy limits.

## What to ask in consultation

Ask whether your problem is dental or skeletal, whether camouflage is a compromise, what happens to your profile, and whether a hospital jaw-surgery opinion is recommended.

### Frequently Asked Questions

Can an underbite be fixed without surgery?

Mild to moderate underbites that are mainly dental rather than skeletal can often be improved without surgery, using fixed braces or aligners with Class III elastics to procline the upper front teeth, retrocline the lower ones and coordinate the arches. This approach is called camouflage, and it changes how the teeth meet rather than where the jaws sit. In growing children an anterior crossbite is sometimes corrected early with an upper removable appliance or a protraction facemask. Severe skeletal underbites, or cases where the facial profile is the main concern, usually need an orthognathic assessment.

What does camouflage mean?

Orthodontic camouflage means moving the teeth to compensate for an underlying jaw discrepancy rather than surgically repositioning the jaws themselves. In a Class III case that typically involves tipping the upper front teeth forwards, tipping the lower front teeth back, and sometimes extracting lower premolars to create the space to do it. The bite can end up meeting correctly and the underbite may no longer be visible, but the skeletal relationship between upper and lower jaw is unchanged. It is a legitimate treatment choice for many patients, provided the compromise is explained and accepted beforehand.

Can aligners do Class III camouflage?

Clear aligners can achieve Class III camouflage in selected mild cases, usually with Class III elastics attached to buttons or precision cuts, and they handle tipping movements and minor arch coordination well. Cases needing substantial lower incisor retraction, extraction space closure, heavy anchorage control or significant root movement are generally more predictable with fixed braces, sometimes supported by temporary anchorage devices. Aligners also depend on being worn around 20 to 22 hours a day. Where the discrepancy is markedly skeletal, neither appliance removes the need for an orthognathic opinion.

Will camouflage change my face?

Camouflage treatment does not reposition the jaws, so it will not change the underlying shape of the face or correct a prominent chin. It can produce a modest change in lip support and in the appearance of the front teeth, because tipping the upper incisors forwards tends to give slightly more upper lip fullness and can make an underbite less obvious when smiling. Patients whose main concern is their facial profile rather than their bite are usually better served by an orthognathic surgery assessment, since camouflage addresses only the teeth.

Is Class III treatment NHS funded?

NHS orthodontic treatment is free for under-18s whose case meets the clinical threshold, assessed with the Index of Orthodontic Treatment Need: dental health component grade 4 or 5, or grade 3 with an aesthetic component of 6 or above. A reverse overjet or an established anterior crossbite usually scores highly, so many children with a Class III bite qualify. Adults are essentially never eligible for routine NHS orthodontics, though where jaw surgery is clinically indicated care may be provided through a hospital orthodontic and maxillofacial pathway. Waiting times vary considerably by area.

What is the main risk?

Moving front teeth beyond safe gum and bone limits, which can affect stability and periodontal health. Excessive proclination of upper incisors can cause recession of the gum on the outer surfaces of those teeth, making them more sensitive and difficult to clean. This risk is higher in adults who already have some bone loss from periodontal disease. A specialist orthodontist should review periodontal health before planning significant incisor movement in any camouflage case.

How do I know if my underbite needs surgery or can be camouflaged?

The distinction is made by a lateral cephalometric X-ray, which measures the actual jaw positions. Your orthodontist calculates the skeletal discrepancy and assesses whether moving teeth within safe bone limits can achieve an acceptable bite and appearance. If the jaw discrepancy is mild and the facial profile is acceptable, camouflage is a realistic option. If the discrepancy is moderate to severe, or if the patient is concerned about their facial profile as well as their bite, orthognathic surgery with orthodontic preparation will produce a more complete and stable correction. A specialist orthodontist can advise after reviewing your cephalometric analysis.
