## How to access NHS orthodontics

NHS orthodontic treatment follows a structured pathway: dentist assessment → IOTN scoring → referral → waiting list → specialist assessment → treatment → retention. Understanding each stage helps you know what to expect and whether the timeline works for your situation.

The pathway begins at your regular dental check-up. Your dentist performs an initial IOTN assessment using the Dental Health Component (DHC), which grades malocclusion severity on a scale of 1 to 5. Grades 4 and 5 typically qualify for NHS treatment. Grade 3 is borderline and requires the Aesthetic Component (AC) to also be assessed.

If you meet the threshold, your dentist makes a formal referral to the local NHS orthodontic service. In some areas, practices accept self-referrals — ask when you call. From referral, you join the waiting list for a specialist orthodontist.

The specialist appointment involves a full clinical assessment: photographs, X-rays, and often a digital scan or impressions. The orthodontist confirms your IOTN score, explains the proposed treatment, and confirms whether NHS treatment will proceed. This appointment usually takes 45–90 minutes.

Once treatment is confirmed, braces are fitted at a separate appointment. NHS braces are almost always metal fixed appliances — ceramic, lingual, or aligner options are not available on the NHS. Treatment typically takes 18–24 months for moderate cases.

Adjustment appointments happen every 6–10 weeks throughout active treatment. Regular attendance is important — missed appointments can extend treatment time. At the end of treatment, braces are removed and retainers are fitted the same day, all within the Band 3 charge.

The biggest practical limitation of the NHS route is waiting time. Depending on your area, the gap between referral and treatment starting can range from around six months in lower-demand areas to four years or more in many parts of England. For families where timing around school, exams, or life events matters, this is an important factor when comparing NHS versus private routes.

### Find out where you stand

#### Single Band 3 charge

The entire NHS treatment course — from assessment through to retainers — is covered by a single Band 3 charge (£332.10 in England from 1 April 2026).

##### Eligible patients pay less

Under-18s, patients on certain benefits, and some low-income adults may be exempt from the NHS charge entirely.

###### Same clinical outcomes

NHS metal fixed braces deliver the same clinical results as private metal braces — the difference is choice of brace type and waiting time.

###### Clear pathway from dentist

Your dentist refers you if you meet the IOTN threshold. You don't need to find an orthodontist yourself — the NHS manages the referral.

###### Retainers included

NHS orthodontic treatment includes retainers as part of the Band 3 course — there is no additional charge for initial retainer fitting.

### Frequently asked questions

How do I get an NHS orthodontic referral?

Ask your dentist for an orthodontic assessment at your next check-up. If your IOTN score qualifies, they make a formal referral to your local NHS orthodontic service.

How long does NHS treatment take to start?

NHS orthodontic waiting times vary by region, typically ranging from around six months in lower-demand areas to four years or more in many parts of England. Private treatment usually begins within weeks of your consultation.

What does NHS orthodontic treatment cost?

NHS treatment is provided at Band 3 — £332.10 in England from 1 April 2026. This covers the full course of treatment from assessment through to retainers. Under-18s, 18-year-olds in full-time education, and adults on qualifying benefits are exempt from charges entirely.

What types of braces does the NHS provide?

NHS braces are almost always metal fixed appliances. Ceramic braces, lingual braces, and clear aligners are not available on the NHS.

Does the NHS cover retainers?

Yes. Retainers are included as part of the NHS Band 3 course of care. Initial retainer fitting at de-bond is covered within the original fee.

Can I choose my NHS orthodontist?

Your dentist makes the referral to the local NHS orthodontic service. You may have limited choice of provider depending on which practices in your area hold NHS contracts.

What if I miss NHS appointments?

Consistently missing adjustment appointments can result in being discharged from NHS treatment. NHS orthodontists have limited slots and cannot carry non-attending patients indefinitely.

What happens at the end of NHS treatment?

Braces are removed at a de-bond appointment, teeth are polished, and retainers are fitted the same day. The de-bond and retainer fitting are included in the Band 3 charge.
