## What is IOTN?

IOTN (the Index of Orthodontic Treatment Need) is a scoring system orthodontists use to describe how “medically necessary” treatment might be. It’s less about whether teeth look straight, and more about whether the bite or tooth position could cause problems — like teeth being at higher risk of damage, difficulty biting or chewing, uneven wear, gum issues caused by severe crowding, or certain bite problems.

In simple terms: a lower IOTN need usually means the issue is mostly cosmetic, while a higher IOTN need suggests the teeth and bite may affect health or function. That’s why IOTN is often referenced in NHS pathways and treatment priority. You don’t need to memorise the numbers — the key idea is that there’s a threshold where cases become more “health need” than “appearance”.

This checker uses the same ideas, translated into plain language. It gives helpful guidance, not a diagnosis. A clinician photo review is the fastest way to confirm the true picture and what options are realistic.

### IOTN and NHS braces questions

What is IOTN?

IOTN (Index of Orthodontic Treatment Need) is a clinical scoring system used by orthodontists to describe how much treatment is needed based on the health and function of the bite, not just appearance. A higher IOTN grade indicates greater clinical need.

What IOTN score do you need for NHS braces?

Children generally need a Dental Health Component score of grade 4 or 5 to qualify for NHS orthodontic treatment. Some grade 3 cases may qualify if the Aesthetic Component score is also high. A registered orthodontist must confirm the exact grade at assessment.

Is this checker a diagnosis?

No — it gives an indication of likely treatment need, not a diagnosis, and it carries no weight with the NHS. Only a registered orthodontist can score IOTN properly, and doing so requires examining the bite in person or reviewing clinical photographs and, where needed, X-rays. Measurements such as overjet in millimetres, contact point displacement, and whether teeth are impacted cannot be judged from a set of self-answered questions. Use the result to understand roughly where you might sit, then arrange an assessment with a registered provider before making any decisions.

What if my IOTN score is below the qualifying threshold?

Falling below the NHS threshold does not mean nothing can be done — it means NHS funding is unlikely, so treatment would be private. Mild to moderate crowding, spacing, and alignment cases are exactly the sort of thing clear aligners and fixed braces treat routinely in UK practices, and these cases often finish faster than more complex ones. A clinician-reviewed photo assessment is the practical next step: it clarifies which appliances suit your case and gives indicative private costs so you can budget before booking a consultation.

Do adults qualify for NHS braces?

Adults are essentially never eligible for NHS orthodontic treatment. NHS funding is directed at under-18s who meet the clinical threshold — IOTN Dental Health Component grade 4 or 5, or grade 3 with an Aesthetic Component of 6 or above — and for those patients treatment is free. Adults are only considered in rare hospital-based cases, usually where orthodontics forms part of jaw surgery or a cleft or craniofacial care pathway. Where an adult is accepted, the NHS Band 3 charge of £332.10 in England applies. In practice, almost all adult orthodontics in the UK is private.

What is the difference between IOTN grades 1 to 5?

Grade 1 is little or no need for treatment; grades 2 and 3 represent mild to moderate need; grade 4 indicates a great need; grade 5 is a very great need. NHS funding is usually available from grade 4 upwards, though local criteria can vary.

How accurate is this IOTN checker?

It is a reasonable indicator of which band you are likely to fall into, but it is not a substitute for a clinical score. The checker turns the main IOTN criteria into plain-language questions and estimates a band from your answers, which works well for clear-cut cases at either end of the scale. It is least reliable around the grade 3 boundary, where NHS eligibility also depends on the Aesthetic Component and small differences in measurement change the outcome. Only a physical examination or clinical photo review by a qualified orthodontist gives a definitive grade.

What should I do after completing the checker?

Book a clinician-reviewed photo assessment as the next step — it turns an estimated band into practical, case-specific guidance. Upload a few guided photos of your teeth and bite through Virtual Consultant Online and a registered clinician reviews them, outlines which treatment options are realistic, and gives indicative costs where treatment would be private. If the checker suggested you may meet the NHS threshold and you are under 18, also speak to your own dentist, who can refer you for an NHS orthodontic assessment. The two routes are not mutually exclusive.
